Care planning offers many benefits to enhance your family member’s quality of life when faced with a terminal illness. The plan may include short-term decisions about medicines and support arrangements, and it may include long term planning to prepare for changing needs.

CARE COORDINATION
 

We assist with the overall transition of care coordination. We start by assessing patient needs and goals then create a proactive care plan with partners, providers, patients, & families.

 

REFERRALS

We offer an easy way for families to request help with an issue or concern, especially if you don’t know which office or department to contact. A DAP Referral works strategically to ensure optimal quality of life for you or your loved one

CLIENT AND FAMILY EDUCATION & TRAINING
 

If you’re exploring the idea of a caring for a loved one, the DAP Client & Family Education Training can give you a perfect introduction. This short course covers the basics, helping you assess your responsibilities and ensuring your loved one receives the care s/he needs and deserves.

 

HEALTH CARE LIAISON

We professionally serve as an advocate for patients and/or family members navigating an increasingly complex healthcare system, reducing their stress and giving them the tools to make informed healthcare choices. 

 

SENIOR LIVING PLACEMENT ASSISTANCE
 

You can do all the research and make all the phone calls yourself, it can be tiring and tedious work.

That’s where The DAP come in. We  work with seniors and family members to help to determine what type of care is needed and what types of facilities are available.
